- Submit a Concern
We take concerns about the conduct of Ontario Certified Teachers (OCTs) seriously.
Concerns can be raised in a few ways:
- Directly with the teacher or their employer: In some cases, it may be appropriate to speak to the teacher or the school principal first and see if the issue can be dealt with at the school board or employer level.
- By talking to us: Call our Investigations and Professional Conduct Department at 437-880-3000 or toll-free at 1-833-966-5588 (in Canada and the U.S.A.).
- By submitting your concern: If the issue is not resolved, information regarding a concern can be filed with the College. Concerns are screened to determine that it is appropriate for the College to investigate and that they meet the requirements under the College’s governing legislation.

Submission checklist
Anonymous concerns will typically not be investigated. If you have concerns about multiple OCTs, you must submit a separate report for each OCT.
In your submission, you must include:
- your name
- your address, phone number and email address
- the nature of your concern
- the name of the OCT
- any other supporting information.
Are you an employer with a concern?
If you employ an OCT and you have a concern related to their conduct or fitness to practise, do not use the intake form below. Contact us directly or read about your Duty to Report.
Online concern form
The following form will ask a series of questions related to your concern.
All information you provide in the Online Concern Form, along with all documents attached to the Online Concern Form and any documents and information later forwarded to the College as part of your concerns, may be disclosed to the OCT as part of the College's legal obligation to disclose all relevant documents. This may include any personal, confidential, and medical information provided as part of your concern.
Please note: If your concern does not relate to the professional misconduct, incompetence or incapacity of an OCT, the College will not investigate the matter further.
