Online Complaint Form


The Ontario College of Teachers investigates complaints of professional misconduct, incompetence and incapacity against College members. The Ontario College of Teachers Act defines a College member as every person who holds a teaching certificate of qualification and registration, subject to any term, condition or limitation.

If a complaint does not relate to professional misconduct, incompetence or incapacity or if it is frivolous, vexatious or an abuse of process, the Investigation Committee will refuse to consider or investigate the matter.


To protect complainants and members who are subject to complaints, the College will not comment on a complaint or an investigation until it has been referred for a public hearing.

Collection of Information
We collect personal contact information, like names and email addresses, when complaint forms are submitted. The information you provide is used to process and respond to your complaint. The College does not investigate anonymous complaints.

Commitment to Data Security
Your personal contact information is kept secure. Only authorized College employees or contractors such as our legal team (who agree to keep information secure and confidential) have access to this information.

Online Complaint Form

The following form allows you to register a complaint against a member with the College. Please note that you will be asked to answer a series of questions to determine the most appropriate course of action and that, based on your answers, it may be suggested that you pursue a course of action other than a complaint. Regardless of the automated suggestion, you can still choose to submit a complaint.

If you are an employer of a member of the Ontario College of Teachers and would like to submit a report to the College, please do not use this online complaint form. Please refer to the Duty to Report section of the College’s website for information on submitting a report as an employer, or contact the College directly to receive further instruction.

Is your concern related to an allegation of physical or sexual abuse of your child, a student or former student?

Are you a member of the College who wishes to submit a complaint about the conduct of another College member?

Do you have concerns about the content of your most recent Teacher Performance Appraisal?

Is this complaint related to concerns that a colleague has made allegations to a supervisor without informing you?

Do you believe that you have been unfairly targeted, have been the subject of bullying or have been otherwise mistreated by another member of the College?

Is your concern related to an incident or incidents that have involved a member being investigated by police or a child protection agency?

Is the member employed by a publicly funded English, French or Catholic school board in Ontario?

Are your allegations related to the treatment of your child or a student?

Have you spoken with the school’s principal regarding your concerns?

If you belong to OTF, please contact your Federation representative. These matters may be more appropriately addressed by your Federation.

Please note that there are certain circumstances under which an employer, such as a school board, must notify the College of a member’s actions. We suggest calling the College at 1-888-534-2222, ext. 582 for information before submitting a complaint.
