Remove Former Name
Description
You can request to have your former name removed if:

there are extenuating circumstances such as a threat to your personal
safety, reasons of personal dignity, including gender identification or
gender expression. If this applies to you, please provide a signed letter
detailing the name(s) you would like withheld, the reasons why and your
application/registration number. Additional supporting documentation may
be required.
The Registrar has the
authority
to permit the deletion of a former name from the register, upon request by
the member, provided the member has never taught under that name. To make
the request, a member must provide all of the following:

a current criminal record check showing all names, including the name the
member is requesting to have removed

a letter from current and previous employers verifying they have never
taught under the name they want removed

a completed Request to Remove Former Name from Public Register and an
affidavit from the member attesting that information in the form is the
truth
- a $25 fee.
