Concerns about College members may be submitted via the form below. Here are a few things to keep in mind before getting started.
Confidentiality
For reasons of confidentiality, we do not comment on any concerns, complaints, investigations or the complaint resolution process.
Collection of Information
The College does not typically investigate concerns raised anonymously. Our online form will ask for your personal information.
Data Security
Only authorized College employees or contractors (our legal team, for example) will have access to personal information. Names of students, victims or vulnerable witnesses will be redacted from all public documentation.

Getting started
The following form will ask a series of questions related to your concern.
All information you provide in the Online Concern or Complaint Form, along with all documents attached to the Online Concern or Complaint Form and any documents and information later forwarded to the College as part of your concerns, may be disclosed to the member as part of the College's legal obligation to disclose all relevant documents. This may include any personal, confidential, and/or medical information provided as part of your concern.
Please note: if your concern does not relate to the professional misconduct of a College member, the College will not investigate the matter further.
If you belong to one of Ontario’s four teacher unions (ETFO, OSSTF, OECTA or AEFO), you may wish to consult your union representative about your concerns before submitting them to the College.

Please note that there are certain circumstances under which an employer, such as a school board, must notify the College of a member’s actions. We suggest calling the College at 1-888-534-2222, ext. 582 for information before submitting an expression of concern.
