Replace My Membership Card

Your membership card is mailed annually to the address the College has on file, along with an updated Certificate of Qualification and Registration once the annual membership fee is processed.

If you require a duplicate or replacement card, you can:

·      call Client Services at 416-961-8800 (toll free in Ontario: 1-888-534-2222)

·      fax your request to Client Services at 416-961-8822

·      mail a signed request along with your registration number to:
